Responsibilities:Conduct accurate and holistic physiotherapy specific assessment for clients during admission, periodic care plan reviews and dischargeSelect appropriate and specific outcome measures to enable evaluation of therapy progress and outcomes in a timely manner to guide clinical decision makingFormulate person centered and SMART goals in collaboration with clients and relevant stakeholdersDesign plans and conduct physiotherapy interventions for clients in a person centered manner, that is also evidence basedDocument assessment results, therapy plans, interventions, goals and outcomes measures according to guidelinesCommunicate assessment results, intervention plans, goals, outcomes and discharge plans to clients and relevant stakeholdersEnsure proper use of equipment including technological-assisted rehabilitation equipmentFormulate discharge and continuity of care plans based on clients’ performance in consultation with relevant stakeholdersFacilitate community integration through collaboration with other community partners as requiredEmpower clients and caregivers with methods to enhance health, mobility and functions through self-management strategies outcomes.
